[Axiom-developer] --patch-47

Patch-47 is up. The primary changes are:

the axiom tutorial book was published at lulu.com (ISBN 1-4116-6597-X)
src/doc contains both the tex source and a pdf of the axiom tutorial book.
it should be available on amazon in about 8 weeks.

changes for the mac version were back-ported.
the mac port still does not work.

cleanup and documentation changes.

i'll be away until the 15th.
