a2ps 4.12 on windows NT

1. Path lookup on network drive, starting with "//" seems to fail.
2. Cannot handle "\" & "/" together as directory separator. For example, if
my LibraryPath use "/", but my input file contains "\", and DIR_SEP was
defined as "/", then the input file cannot be found. I guess DIR_SEP just
shouldn't be used in getting input file name.
3. --medium=Legal --setpagedevice=PageSize:"[612 1008]" can not combined
with --center-title="title-text". An "invalid argument" error is displayed
but I don't see why it isn't valid.
