a2ps only prints blank pages (or none at all)

I'm trying to use a2ps to pretty-print some C++ source code, but a2ps
keeps printing only blank pages. On some printers, it doesn't print at
all. All my other printing utilities are working fine, so I know my
problem is with a2ps. Can you give me guidance on troubleshooting this
problem?

Attached is 
1- the file I'm trying to pretty print,
2- the postscript file generated by a2ps,
3- the output of a2ps with the --debug option
