Summary:  make epsilonlex more coherent with epsilonyacc

Original Submission:  In a scanner for a language XXXX, throw an exception
XXXX_scan_error instead of the current "generic" scan_exception.

Also throw XXXX_scan_error instead of out-of-bound when looking up a character 
not belonging to the alphabet.
