Thank you for for you email.  Due to the high volume of emails, it may be 
several days before you receive a reply.  If you need immediate assistance 
regarding a DIVER level certification, please call the research department 
directly at 1-800-729-7234 x2430 or 1-949-858-7234 x2430 Monday through 
Thrusday 7am-5pm pst and Fridays 7am-4pm.  If you have a question regarding a 
MEMBER level (Divemaster or Instructor), please email address@hidden or call 
the Membership department at x2495.  

Answers to some common questions :
 
If you have not received your Diver level PADI certification card:

Please note many cards are sent directly to your diveshop or instructor upon 
their request.  Please check with them first, if they do not have your card, we 
will look into it as soon as possible

If you would like information on how to obtain a replacement Diver level PADI 
certification card:

You can order one online at https://www.padimembers.com/sforms/replacement/ if 
you have a credit card and a digital photo of yourself.  If you do not have 
these, or need a name change, you must order through regular mail.  Here are 
the instructions:

In order to obtain an updated certification card, please submit a written 
request along with the original name on your card, student number (if 
applicable), date of birth, level of certification, certification date, 
photograph (passport sized, no dark glasses) and $25 U.S. processing fee (Make 
personal checks payable to “PADI”, money orders, cash and credit cards 
w/expiration dates are also acceptable). Normal processing is 2-3 weeks from 
the time we receive your request in the mail.   Priority Processing (48 -hour 
processing) is also available for an additional $10.    If you would like this 
service, please indicate "Priority Processing" in your request.

You can obtain Project Aware version of your certification card for a minimum 
donation of $5.00.  All proceeds go to the Project Aware Foundation, a 
non-profit organization dedicated to protecting the underwater environment.  If 
you would like this option,  please indicate the donation amount should be 
directed to the Project Aware Foundation.

If you have had a recent name change, please provide copies of any name change 
documents.  (e.g. copy of marriage license, court documents, or copies of 
driver’s license, passport, or social security card with new name on it)

If you are unable to provide us with any specific certification card 
information, please include your date of birth and approximate year of 
certification.  To ensure prompt processing, please be sure to indicate the 
appropriate mailing address of this card.
