Additional Locale information

Hello,

Below is a patch (and ChangeLog entry summary) that adds additional
information to locale files: adding fields to LC_ADDRESS, LC_TELEPHONE,
and LC_IDENTIFICATION.  See ChangeLog for details.

This patch is against glibc-2.2.5.
