Thanks for the additional information.

Unfortunately, I'm still unable to reproduce the problem, so I think I
need more info.

Here's what I did:

    emacs -q
    M-: (add-to-list 'Info-default-directory-list "d:/usr/info") RET
    C-h i

The directory d:/usr/info on my system holds many Info files and also
a DIR file.  However, the buffer displayed after "C-h i" above has the
expected contents: first the DIR file from d:/usr/info, then the DIR
file from the Emacs installation (which is in d:/usr/ntemacs on this
machine, so it's not in c:/emacs).  I don't see any duplication of
menu entries.

Could you please repeat my experiment on your machine (only with the
names of directories you add to Info-default-directory-list replaced
with the names from your system), and then see if you still get the
Emacs's DIR file twice?

Also, is it possible that one of the directories you add to
Info-default-directory-list has the Emacs entries in its DIR file?
