Details:

The PIBHEX output format (and probably others) is buggy.  The following
segfaults:

data list /x 1.
begin data.
1
end data.
formats x(pibhex4).
list.

I'm revamping the input/output right now and will add thorough test cases for
this and other formats, so this bug is really just a note to make sure I don't
forget about it.
