Summary:  NSRulerView can draw 'zero' in wrong location

Original Submission:  When the document view's bounds are not at coordinate 
(0,0),
the ruler view is misplaced. The attached patch corrects that.

It also corrects some border cases when a hash or a label
could be drawn out of the baseline; adds calls to 
invalidateHashMarks in some places the cached values
should be invalidated and removes an ivar that was only
being used in one method and an unused macro.
