Summary:  Command does not work with Shift

Original Submission:  If I press Shift (and keep it down), then Command (and 
keep it down), then 'q', the library thinks I'm pressing Shift+q, but I'm 
pressing Shift+Command+q !!

I tend to press Shift before Command when I build this sequence of keypresses 
and the library is totally broken if it can't cope with it.

It used to work.

There are other bugs, for example if I press 'Command + Shift + l', then I 
raise the Command but keep the Shift pressed, then I press k, the library 
thinks I'm now pressing Command + Shift + k, but Command is no longer pressed 
so the library is wrong!

It looks like the library can't cope with raising and pressing Command while 
Shift is pressed.

This used to work so this is a regression.
