grub-mkrescue --image-type=floppy floppy.img
If you want to have a custom grub.cfg there then:
mkdir -p overlay/boot/grub
emacs overlay/boot/grub/grub.cfg
(or cp etc.)
grub-mkrescue --image-type=floppy --overlay=overlay/ floppy.img
