A workaround is to do

\version "2.10.19"
\include "deutsch.ly"

\new Staff = "foobarbaz" \with {
 fontSize = #-3
 \override AccidentalSuggestion #'cautionary-style = ##f
\override AccidentalSuggestion #'font-size = #-5 % 2 steps smaller than fontSize!
 \override StaffSymbol #'staff-space = #(magstep -3)
}
{\set suggestAccidentals = ##t cis''}
