I hope that you realize that the manual for 2.10.33 is almost a
year old, and the docs have undergone massive improvements in
2.11.

I recall fixing some sort of problem like this over a year ago,
but it was probably only in the 2.11 docs.  Could you try printing
out the same page in the 2.11 pdf?

Page breaks are handled automatically, and are pretty much out of
our control.

Could you give me section names, instead of page numbers?  If you
do that, I'll check to see if these index entries are present in
the 2.11 docs.
