I tried adding dynamic marks in the first multi-voice measures for the left and the right hand, but it's not enough. I'm afraid I'll need them in every measure. Also, I'll need to make them transparent, or the score would be hard to read. It's easier for me to edit the resulting MIDI-file.
Heikki reported several MIDI bugs here: http://lists.gnu.org/archive/html/bug-lilypond/2011-09/msg00987.html and he fixed one closely-related bug, as of version 2.15.14
