Hmm, supposing the output is well-balanced, one can also remove
indices using a teeny tiny bit of XSL stylesheet, instead of hacking
the makeinfo sources and removing something that may be of use to
somebody in the future.  I'll look into this.

BTW, Arnold Robbins also used the DocBook output when writing his
'Linux Programming By Example' book. ;)
