[VCDImager Bugs/Devel] cd recording plugin, second try

Since I'm going on holiday for a week tomorrow, I am putting the
latest version of my cd recording patch for vcdimager on line, in
case someone wants to give it a try.

It now also works with my plextor writer (though I don't know why)
and can recover from a buffer underrun when writing to CD.

There is working code for real-time input from a pipe in the patch,
but that I disabled it for now since it conflicts with the normal
operation and I'm doing it differently later.
