Perhaps we could just add a command-line option to cvsdiff that would
cause it to behave as you propose?  Or even make it the default
behavior (even though I don't quite like the change in behavior, it's
not like it would affect myself, since I always extract diffs from the
top of the tree anyway).

Also, we could add a command-line option that would just prefix all
pathnames in ^Index: lines with the given argument (a different
argument could extract the prefix from the cwd, as your script does).
fixpatch would take care of propagating the changes in Index: to the
actual pathnames in the diffs.
