Follow-up Comment #1:

You have to save the command set after you have created a new menu item.
Likewise, if you add a menu item, you have to do this.
It is stupid, I'm sorry. I'll try and fix it. The problem may be that all the
commands are added at program startup, and you wouldn't want it stopping after
adding each one to ask if you want to save your command set.
but it is stupid.

Your second problem is the state of the art: the display is crude, it does
let you know you have a ff at that note. You could place it the graphic lower
down by fiddling with the gy value in the script, which determines the y
position (relative, I think to the note). But improving the display appearance
is low priority: for me the printed page is crucial. (For others the MIDI
output!).
