We need to assign a program to a channel. Or send a program change if  
two staffs are sharing the same channel but using different programs.  
The code us there to assign a program to a channel but it is commented  
out for now. I believe there is also a commented out example of how to  
send a program change. I think I wrote a function in midi.c to get the  
program number. Program number is dirived fro
 the instrument name unless this has been overrided with a number.
