I have found a bug for the chord insertion with the sustain pedal. If
the selected note length is the same as the measure length and there are
empty measures following, then while holding the sustain pedal down
notes are not inserted as chord, but each note in a new measure.

You can reproduce this as follows:
* in a new score, create a few empty measures via AddMeasure
* select duration as whole note
* press the sustain pedal and play some notes
Those notes will each be inserted in a new measures instead of as chord.
