allout patch - remove gpg workaround now that gpg is fixed

this patch removes a workaround allout had for a recently fixed
pgg-gpg-process-region bug.  the other changes are in docstrings -
clarifications prompted by some questions from a user.

the patch and changelog entry are attached, and the changelog entry is
also below.

        of gpg-agent, now that pgg-gpg-process-region correctly honors
        passed-in passphrase.
        (allout-distinctive-bullets-string): Add info about bullet
