CVS Ignore files for Cocoa build on OSX

After a build, CVS is confused as follows:

Locohost.local:~/MIRROR/emacs-CVS % cvs -q up
? lib-src/b2m.dSYM
? lib-src/cvtmail.dSYM
? lib-src/digest-doc.dSYM
? lib-src/ebrowse.dSYM
? lib-src/emacsclient.dSYM
? lib-src/etags.dSYM
? lib-src/fakemail.dSYM
? lib-src/hexl.dSYM
? lib-src/make-docfile.dSYM
? lib-src/profile.dSYM
? lib-src/sorted-doc.dSYM
? lib-src/test-distrib.dSYM
? nextstep/Emacs.app
? src/prefix-args.dSYM

The .dSYM files should either be deleted if they're not needed after a build,
or ignored.  nextstep/Emacs.app is an output, so it should just be ignored.
