    Remove codepage setup code from mm-util
    
    * lisp/gnus/mm-util.el (mm-codepage-setup): Remove.
    (mm-codepage-iso-8859-list): Remove.
    (mm-codepage-ibm-list, mm-setup-codepage-iso-8859)
    (mm-setup-codepage-ibm): Remove.
    (mm-charset-eval-alist): Remove the code pages from the
    default value.
