Nope, my error. $1 gets clobbered by the second replacement. So you'd
want what you wrote or somewhat shorter:
# hyperlinks
s/\[\[([^]]*)\]\]/
my ($l, $o) = ($1, $1);
$l =~ s: :_:g;
"[[file:$l.org][$o]]"/gex;
