Carbon/MacOSX File Dialog Patch

To try to complete the MAC_TODO's that are left around I'm trying to 
tackle the easier ones, and that includes the file dialogs.  I did this 
once before with the pre-Navigation Services stuff, and also with the 
applescript stuff I mention in the patch; but this seems more 
definitive.  It seems to work, with the following exceptions

- There is an internal double free that I have to debug (but it doesn't 
seem to cause any problems)
- It doesn't correctly determine whether to use a save or an open 
dialog, so it greys out some of the files that it shouldn't.
- I'm not sure how it will behave on 10.1 systems (since I no longer 
have one)

I'd like people to test it out before I check it in, because it does 
modify default behaviour.  Just use the menu (or tool-bar) to try it 
out.
