You mean that you don't see the problem with the original (slow)
version of create-fontset-from-mac-roman-font?

OK, I checked this last night and found that I could (instably) reproduce the problem on the original version (without your patch) as well. I've reduced the problem to a small amount of code. I'm attaching a .emacs, and the file that is used to define the fonts. I can reproduce this with the unpatched CVS Emacs compiled as below (this is the Japanese CarbonEmacs binary, without their site-lisp) as follows:

When loading, it should define a couple of fontsets.

Then, execute (change-frame), AND DO SO A SECOND TIME.

After the first time, everything is fine. After the second call to change-frame, the scroll-bar position is screwed up - it's far off to the left.

I get this issue consistently, at least on my OS X 10.4.1 system.

At this point, let me reiterate that when using variable-width fonts, the scroll-bar position is generally bad (not always as bad and inconsistent as above). You told me earlier on that Emacs requires the frame to be of certain widths - no good, but fair enough. But then, the scroll-bar should be on the right-hand side of the frame, and not somewhere 10 pixels to the left. If there is some background without text between the scroll-bar and the window content, alright, that wouldn't look bad. But as it is right now, it's just plain ugly. It'd be really nice if we could get this to look and feel right.
