OK, I can see it will never work in the help buffer.  I've installed your
fix (almost):  help-insert-string just sets help-xref-stack-item rather than
call help-setup-xref.

If it still doesn't work, then please fix it as you see fit.  Although
appropriate here, I think this approach is inappropriate in many cases
e.g `C-h v' could re-create a Help buffer with an out-of-date value which
could be very confusing and generally not what is wanted.  So I'm not sure
I agree with Stafan's idea of a general approach.  Currently this seems to
be a special case.
