Even the variable index is just around 700 lines long - it might be a good idea to rethink the decision from back then. On today's machines, making those links clickable shouldn't take very long, and certainly not as long as it takes for the user to go a) unterstand how the index (doesn't) work, b) go back to the main contents page and c) find and select the appropriate chapter.
