I'd tend towards fixing them as we come across them, as SIZET_FMT 
is an annoyance more than anything else.

Whether we want to use the solely boost::format subsituters %1%,
%2% etc instead of %d, %u and friends is another question; any of
the latter is strictly incorrect for size_t, so a neutral identifier might
be more appropriate.
