According to the xv website [1] a license fee must be paid for commercial use. According to a Debian bug report [2] distribution of modified binaries is not allowed, which is why it was removed.
The package that refers to it is x11-common, one of the X.org binaries. The reference is in Conflicts, meaning that xv is removed when X.org is installed.
