[Gnumed-devel] patient data input screen prototype as promised

Rough gui enclosed as png file.

Obviously the data is totally random and dummy!!!!

Still lotz of room to add more fields.

Each plugin like this will bring its own complete toolbar when used for 
multiple patients and data entry, and live on the bottom tab. Could be used 
as plugin for a billing system.

In gnumed at first will probably just display data imported from the billing 
client or other demographic datasource.

Patient display only without the white viewing area for multiple lists will 
live on the patient tab for display purposes not data entry.

I've forgotton some fields (like mobile phone!!! - just noticed). Need to add 
some country specific identifiers, or a few fields which are user defined.

Feedback quick please, suggestions welcome.
