- the latter would need to be clin_root_item *child* items but
  you knew that
- the internal consistency of putting the encounter at the top
  of the hierarchy seems violated, episodes and health issues
  can span several encounters

The other suggestions sound OK. Do you intend to implement
them for the web client ?
