"Contact numbers and preferences" would imply phone numbers (residential, work, mobile). This may or may not imply fax number however only rarely have I ever faxed anything to a patient, only a signed lab request.

"Contact information and preferences" is more general, and should imply residential and mailing addresses, as well as email addresses and phone numbers.

Among the phone numbers, it would be nice to be able to record the order of preference (1,2,3 etc) and/or to be able to designate something helpful about a schedule. The ability to record "home" and "work" are helpful but not if the patient works nights and is at home during the day.
