You may want to upgrade your GNUmed version like so:

To add an address you need to fill in all the fields. This
is much improved in 0.3 (not yet available for
Debian/Ubuntu).
You will have to select an appropriate province for the city
Riche Terre. If there is no province available in GNUmed use
"StateNameStub" or better yet
"state/territory/province/region not available".
v0.3 allows to edit the list of provinces yourself.
