You need to call
msgfmt -v --statistics -o gnumed.mo pt-br.po
for that you need the gettext package installed.
Then copy the gnumed.mo into:
/usr/share/locale/pt_BR/LC_MESSAGES/
A system-wide GNUmed installation should then pick up that
file and use it - as long as your system is set to pt_BR
locale.
If you are running from a CVS tree you need to copy
gnumed.mo to
client/locale/pt_BR/LC_MESSAGES/
