It is much harder than "VERY hard".

Essentially it is a binary decision. How else would you
implement "automatic, implicit linking of code and episode"
in parallel to "manual, non-automatic, explicit linking of
code and episode" ?

How shall one guarantue both of "same episode name always
has the same code, regardless of which patient" and "same
episode name can have different codes depending on which
patient".

If someone knows - I'd like to know, too !   :-)
