structured information that will be able to be managed in

for example, which other providers the patient has seen and may still be seeing.

Therefore, it may make sense to keep the structured data clustered together, 
with the existing free text notes field relocated either above or below the 
structured information.

I would suggest that "Free text notes" be moved to the top, because it is at 
the top that any "special" and potentially dynamically-changing information may 
be most warrant the user's attention.
