I have modified the EMR tree to load sub-node data upon
expansion of said node. This means that episodes are only
loaded when an issue node is expanded and encounters are
only looked up when an episode node is expanded.

The drawback is that nodes cannot communicate the state of
their sub nodes (say, issues cannot be set to bold when
containing an open episode) before they are first expanded.

This seems to speed up (re)loading of the EMR tree quite a
bit -- at the expense of requiring some time when expanding
nodes. On local databases it doesn't make a difference
speedwise (but it *does* have the above drawbacks).
