[Gnumed-devel] Audit log enhancement

Since the main interaction to know about in logging is the interaction of a 
user and the patient whose record the user added to (or altered), it would be 
informative to add, to the current audit log GUI display columns,

- the patient's currently-active name
- perhaps the patient ID number

If the audit log includes, inits select, tables which might not be patient 
related (for example, some inbox records might not be patient-related) then the 
query would have to provide a way to handle a display of non-patient results 
like

        N/A
or      n/a
