Details:

While working on the migration to Python3 and Tryton 4.2, I was checking the
__register__ code in different classes.

There is quite a bit of code that has to do with upgrading from older GNU
Health versions. 

I think it is sane to remove the migration code previous to version 3.0. That
leave a leaner and cleaner codebase.

GNU Health 3.2 introduces quite a bit of changes (Python3, Tryton 4.2,
Federation, ... ) and I think it's good to take the opportunity to do some
previous house cleaning.

For users that are still in versions older than 3.0, they can upgrade to 3.0
and from there, migrate to GNU Health 3.2
