Not sure to have fully understood.
IMHO, static roles defined by flags may not describe facts as they are. The role of a party (not necessarily one and often simultaneous) is defined by which records are accecible by whom and when. A good management of access rules is enough. Though, some flags may be usefull in products to enhance safety.
People id may become a major concern if and when GH becomes used in multinational datawarehousing systems. Some sort of soundex search and hashes will probably be incorporated. Note that several countries are paving the way to close many hospitals that are going to be used only for surgical procedures. In France, for instance, it is predicted that there will be no more than 3 regional hospitals in 25 years. With most treatments conducted at patients' homes, datawarehousing, useful to GH today, will be mandatory in a few years and patient idenfication will have to be tougher on desambiguation.
