As usual, I stumbled upon the answer just after posting :-) In
~/.emacs:
(add-to-list 'allout-mode-leaders '(perl-mode . "###_"))
or whatever strikes your fancy. Then in your .perl file, use comments
like this:
###_. or
###_#1 or
###_, etc.
