The only ways that I know of that you can permenantly lose a version of a file is to go to the server and do a 'rm -f file,v'; ie, remove the repository file itself, or use 'cvs admin' commands to obsolete revisions of files. The whole point behind CVS is that you retain the modification history of the files forever.
If the file is removed from the head branch using 'cvs remove', the same method that you used to recover it from your integration branch would recover it in the head branch.
