Consider the following sequence of events:
Dave borrows item A
Mary make a request for item A
Dave returns item A
Fred (tries to) borrows item A.

What Koha will do when trying to issue item A to Fred is, it will detect that
there is an outstanding request by Mary and ask if the issuing to Fred should
be allowed. If you answer "yes" at that point, it will then ask if Mary's
request should be cancelled. If you answer "yes" to that, the request won't
actually be cancelled.
