Strange! This is one of the fonts where the .mf file should 
be generated automatically on the fly. I tried with testfont
and got:

~> tex testfont
This is TeX, Version 3.14159 (Web2C 7.3.1)

(/afs/e.kth.se/system/systype/@sys/mpkg/tetex/1.0.7/texmf/tex/plain/base/testfo
nt.tex 
Name of the font to test = cmbxti8
kpathsea: Running mktextfm  cmbxti8

This is METAFONT, Version 2.7182 (Web2C 7.3.1)

kpathsea: Running mktexmf  cmbxti8
mktexmf: /var/tmp/texfonts/source/jknappen/sauter/cmbxti8.mf: successfully 
generated.
...

Please verify that you have the file b-cmbxti.mf installed:
~> kpsewhich b-cmbxti.mf

If it is, then 
~> mktexmf cmbxti8
should generate the desired .mf file.

We run teTeX 1.07 here, but I don't think the handling of
the "Sauter" fonts has changed for a long time.
