Strange! This is one of the fonts where the .mf file should
be generated automatically on the fly. I tried with testfont
and got:
~> tex testfont
This is TeX, Version 3.14159 (Web2C 7.3.1)

Please verify that you have the file b-cmbxti.mf installed:
~> kpsewhich b-cmbxti.mf
If it is, then
~> mktexmf cmbxti8
should generate the desired .mf file.
We run teTeX 1.07 here, but I don't think the handling of
the "Sauter" fonts has changed for a long time.
