Well, usually that is all staves in the same PianoStaff / GrandStaff and 
nothing else. Not that hard, really :-)

No, I prefer making routines allowing lilypond to typeset accidentals 
automatically - otherwise i.e. transposing is impossible.
I cannot see any real problems. Ofcourse one has to state if two voices in the 
same staff applies to different instruments or the same instrument - but so 
what?
