bugs in dynamic spacing

I know that we need to do a fair amount of tweaking of #'padding to 
make good output, but I think these are bugs -- my understanding is 
that Lilypond aims to correctly space dynamics and notes.

On lilypond/fink 2.3.8, in the third bar the \mp is touching the sharp 
sign of cis
In addition, the \setTextCresc crescendo is printed on top of \mp.
