unlikely, I think staff-space, which at 20pt staff-height = 5pt = approx 1.7 mm.

Could it be a combination of values?

I just tried that "bug" example with staff size of 20 and 15. Top-margin, head-separation, and bottom-margin were 5, 4, and 6 in both examples. ly/paper-defaults.ly contains
    top-margin = 5 \mm
    bottom-margin = 6 \mm
    head-separation = 4 \mm
    foot-separation = 4 \mm

I think that would be a bug. It's possible that something's broken in the scaling between physical units and staff spaces.
