1. go to web page.  log in if you're not logged in (top right
corner)
2. look at the "created by me" issues.
3. if it's been pushed, click on the round "x" button.

alternate step 3: click on issue to load the specific page, then
click on "edit issue" (top of the left bar), then click on "close
issue" (or maybe just "close") and the "update".
