The ly2dvi script is fairly limited when comes to changing the 
page layout. If you want to design the page layout more freely, 
I suggest to use lilypond-book instead, see the documentation and
the example files lilypond-1.x.xx/Documentation/user/latex-*.latex

For minor changes, you could try to put a modified copy of 
lilypond-1.x.x/tex/titledefs.tex in you working directory. 
Note, though, that this solution is less future-proof since 
titledefs.tex may change.
